- Evaluate voluntary external standards to advise the organization on whether these advance or impede the achievement of strategic goals. Examples include service line awards/certifications, Leapfrog and clinical registries). Current clinical registries include National Surgical Quality Improvement Project (NSQIP), Vascular Quality Improvement (VQI), American Joint Replacement Registry (AJRR) hips and knees, American Academy of Orthopedic Surgeons Registry (AAOS) shoulder and spine, and Society of Thoracic Surgeons Registry (STS).
Patient Safety
- Conduct a biennial culture of patient safety survey.
- Ensure that processes are created to address gaps identified in the survey findings.
- Apply root cause analysis (RCA), FMEA, surveillance activities and risk assessments to identify and evaluate patient safety risks.
- Utilize findings from RCA, gap analysis, surveys and audits to design appropriate interventions.
- Assist in reviewing medical record documentation for appropriateness of care and advise leadership accordingly.
- Work with Risk Management department to track and trend patient safety issues, develop plans for improvement and ensure appropriate reporting to state and federal agencies.
Regulatory and Accreditation
- Provide leadership and guidance in the development and implementation of the Quality Management/Patient Safety System that involves all department and services, focuses on indicators related to improved health outcomes and reduction of medical errors.
- Intervene when necessary, involving senior leadership, to hold owners accountable for actions and established deadlines.
- Provide analysis and interpretive guidance of federal and state regulations/standards.
- Establish a continuous accreditation survey readiness process that incorporates accountability and participation by all levels of staff.
- Provide oversight for presurvey, onsite survey, and post-survey accreditation activities.
- Oversee the coordination, submission and monitoring of corrective action plans associated with accreditation and/or certification of service lines.
- Ensure that an electronic document management system is maintained.

• Conducts nutrition assessments for patients at high, moderate, and low risk, and patients requiring artificial nutrition support.
• Implementation and monitoring of Medical Nutrition Therapy interventions.

EDUCATION AND EXPERIENCE

• Must be licensed in the State of Texas or able to obtain licensure within one year of date of hire
• No experience required.
• Prefer 1-2 years of hospital experience.

The Discharge Planner assists the licensed Social Worker, RN Case Manager, Physician, and/or any other member of the healthcare team in coordinating, negotiating, procuring, and facilitating the patient’s discharge plan for the achievement of quality and cost-effective outcomes. The Discharge Planner will work in collaboration with interdisciplinary systems that are both internal and external to the Midland Memorial Hospital. The Discharge Planner will participate in quality improvement and evaluation processes related to the discharge planning.

- Weekend Discharge Planner: Meets with Hospitalist staff on weekends to assist in facilitation of the patients discharge by completing post hospital arrangements. Meets with each member of the case management team on Fridays and Mondays to receive or provide an update to identified group of patients’ plan of care.
- Provides Important Message from Medicare letters to Medicare recipients when needed.
- Able to navigate within the electronic health care record (EHR) to gather clinical documentation pertaining to the patient, current patient demographics, and utilize software application to securely transmit the information as need for outside referral sources for continuation of care.
- Provides accurate, timely, and complete documentation in EHR.

This position is focused on patient and guest check in while providing an excellent guest service experience at the point of entry. The Guest Assistant will greet all guests entering the facility with a welcoming and positive demeanor. They will ensure, if the guest is a patient, confirm scheduled appointment, provide registration onboarding and provide escort to the proper hospital destination. The Assistant will welcome visitors and validate patient name and room number to ensure proper visitor management as well as communication of visitor guidelines. The Assistant will respond to any immediate needs identified, within their capability or obtain the appropriate assistance for those needs outside of their scope of service. The Assistant will be required to be aware of the location of all departments and general operations of the hospital so that knowledgeable answers will be given to guests in a timely manner. The Assistant must exhibit a professional demeanor including making appropriate eye contact, smiling and rising to greet guests as they enter the building. The Assistant should be able to demonstrate active listening and adaptability to the needs of the guest. The Assistant will be required to wear a uniform and maintain good grooming at all times.

Patient Access Registrar’s primary responsibilities are:
1) to gather all pertinent patient information 2) to accurately admit/register or pre-admit patients; 3) to pre-certify all applicable admissions/pre-admissions; 4) to obtain signatures on all appropriate admissions forms if appropriate; 5) to ensure that patient benefits/eligibility are verified and entered into the Hospital’s Information’s System when applicable;
6) when appropriate estimate and collect all co-pay’s/deductible’s that are owed to the hospital to meet departmental requirements; 7) identify and refer patients’ to the financial assistance department at MMH; and 8) to provide a full range of insurance verification functions on an as needed basis as requested by Management.
Additionally, because the Patient Access Registrar carries the responsibility of acting as the Hospital’s primary entry point into the system, all job duties are completed in a manner that is consistent with the highest quality of guest relations which results in a positive experience for the patient and/or his/her family member(s). The position also requires accountability for admitting/registering and/or providing verbal directions to patients and/or family members in any or all the following age groups: neonatal, pediatric, adolescent, and/or geriatric demonstration the appropriate skills to understand and react effectively to the needs of these patients and/or family members.
