This position is responsible for providing administrative and clinical support to the Patient Navigators and will be a liaison for patients in need of multiple services and support. This includes all support needed for high-risk genetic, breast and lung patients. The assistant to the nurse navigators’ primary function is guiding patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. Providing basic client services: follow-up calls, request for information and assisting with follow-up care. Be able to assist with providing care in a variety of situations using solid judgment and critical thinking skills in making decisions. Works as part of a team to promote productivity and a positive working environment. Supports patients and family members who have questions and issues.

 

SHIFT AND SCHEDULE

Monday - Friday - 8:00 AM - 5:00 PM

 

ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS

  • Provides administrative support. By conducting research, handling information requestsand performing clerical functions such as filing, receiving visitors, arranging conference calls, scheduling meetings and setting up meeting rooms.
  • Complete a comprehensive assessment. This includes the initial and ongoing, extensive  collection, analysis and interpretation of data.
  • Synthesize the data collected during the comprehensive assessment to identify problems,to formulate goals, teaching plans and outcomes.
  • Evaluate and report patient outcomes and responses to therapeutic interventions in comparison to benchmarks from evidence-based practice and research findings and plans any follow-up care and referrals to appropriate resources that may be needed.
  • Able to critically think and independently solve various problems or understand when guidance is needed.
  • Able to work with minimal supervision; completing tasks on time.
  • Contributes to the organization and efficiency of the department.
  • Collaborates and communicates well in verbal and written forms with others.
  • Ability to maintain confidentiality of both verbal and written information in which the position may encounter related to both employees and patients we serve.
  • Ability to work in a patient centered, team-oriented environment
  • Desire and ability to become knowledgeable in medical terminology.
  • Demonstrates an ability and willingness to become knowledgeable of the components of patient navigation.
  • Communicates with patients, centralized scheduling, and other staff to schedule patient appointments.
  • Works with provider’s offices to coordinate patient appointments.
  • Assists patient navigators with tasks contributing to the overall performance of the department.
  • Completes audits and reports at Radiation Safety Meeting and Cancer Committees.
  • Answers phone lines for the department and demonstrates the ability to answer questions or refer questions to appropriate sources.
  • Ability to send out reminder letters for communication. Working the incidental findings, special notification and high-risk reminder letters. As well as the critical finding notification folders daily.
  • Collects data and analysis utilization follow-up criteria and reports downstream revenue. Completes the meeting minutes for the Comprehensive Breast Committee and maintains records for NAPBC accreditation

EDUCATION AND EXPERIENCE:

  • Minimum of a high school diploma or equivalent.
  • Associates degree or higher preferred.
  • Bilingual in Spanish is strongly preferred.
  • Customer service expert: telephone and email follow-up beyond compare. Ability to verbally communicate with all levels of the organization and vendors.
  • Must have excellent writing and verbal skills
  • Ability to work efficiently under conditions of multiple deadlines and changing priorities to produce a large volume of high-quality material with meticulous attention to detail.
  • Microsoft Office experience required.
  • Experience in a hospital environment preferred.
