The Manager of Case Management/Social Service will report directly to the Director of Case Management and be responsible for the day-to-day operations associated with Case Management and Social Service departments. Ensuring accuracy and completeness of work, proper staffing and training of employees and adherence to established departmental and hospital policies and procedures. The Manager of Case Management/Social Service will work with the Director of Case Management for the betterment of Midland Health through continuous consistent departmental performance improvement.

ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS
· Provides leadership, direction and support to the Case Management and Social Servicestaff to ensure quality of performance, adherence to organizational policies and procedures, and practice within licensure.
· Works directly with the Director of Case Management to create, implement, evaluate, and revise departmental policies and procedures, contractual agreements, and staff processes and training.
· Demonstrates critical thinking skills when making decisions and analyzing data that are congruent with the hospital’s strategic goals.
· Secures appropriate educational presentations and/or conferences for both non-clinical and clinical to ensure the Case Management and Social Service staff is empowered with the most recent educational information.
· Act as an educational resource to all Midland Health patients, visitors, employees, and physicians.
· Produces timely reports that are professional in appearance, reflect appropriate analysis of the data, and contain conclusions and action plans when needed.
· Develops strong working relationships with other program managers and directors, throughout Midland Health.
· Understands the financial responsibility of appropriate utilization of resources to maintain monthly/annual departmental operating and capital budget.
· Attends departmental, organizational, and community meetings to provide and/or gain meaningful information and to foster strong working relationships all practices involved
with healthcare.
· Maintains knowledge of the day-to-day operations and ability to perform the function of any staff member associated with Case Management or Social Service department.
EDUCATION AND EXPERIENCE
· Current State of Texas License as a Registered Nurse.
· Five years of acute care nursing experience.
· Case management and/or supervisory experience preferred.
· Proficient in Microsoft Word and Excel.
· Preferred certification in Case Management by the CCMC or the ANCC
